What is MOM Dependant's Pass / LTVP application (EP Sponsorship)?

Singapore's Ministry of Manpower (MOM) uses this form for family members of Employment Pass candidates and holders whose pass is under the Sponsorship scheme. It covers a Dependant's Pass (DP) for a legally married spouse or an unmarried, or legally adopted, child under 21, and a Long-Term Visit Pass (LTVP) for a common-law spouse, an unmarried stepchild under 21, an unmarried handicapped child aged 21 and above, or a parent. MOM notes that dependants of other Employment Pass or S Pass applications apply online at go.gov.sg/ep-eservice instead.

Three parties complete it. The foreigner (the family member) and the work pass candidate or holder fill in Parts 1 to 6 and sign their declarations. They then send it to the local sponsor company, which checks those parts, completes Parts 7 and 8, uploads the form with the supporting documents and pays the application fee online. The form states that processing takes within 6 weeks, and some cases take longer.

VergeHR asks each question in plain language, shows only the follow-up questions that apply (state or province lists, identity card numbers, the CDA vaccination part for young children), and prints the answers into MOM's own PDF. It supports names with accents, and can send the form for e-signature by each party in turn.

Who fills it
The foreigner and the work pass candidate/holder (Parts 1-6), then the local sponsor company (Parts 7-8) and any employment agency used.
Submitted by
The local sponsor, online at go.gov.sg/submit-dependants-of-ep-sponsorship, with the fee (PayNow, Visa, Mastercard or Amex). The organisation needs a Corppass account.
Processing
Within 6 weeks according to the form; some cases take longer.
Signatures
The foreigner (a parent signs for a child under 16), the work pass candidate/holder, the sponsor's representative, and the employment agency personnel if one was used.

How to fill out MOM Dependant's Pass / LTVP application (EP Sponsorship), step by step

1 Part 1: Application information

Give an email address MOM can use to contact you, the work pass candidate/holder's FIN, name and alias, any FIN or Work Permit/S Pass number the foreigner has held, the duration applied for (from 1 month up to the work pass holder's pass expiry), and the relationship. The instructions say to enter 'Not applicable' or 'N.A' rather than leave a field blank.

2 Part 2: Foreigner's personal information

Enter the foreigner's particulars exactly as in the travel document, without salutations. Chinese, Indian and Malaysian nationals choose a state or province of nationality, and people born in or originating from China, India, Malaysia or the United Kingdom choose a state or province of birth or origin. Identity card numbers are asked for Bangladeshi, Chinese and Malaysian nationals. Add the travel document details and the foreigner's email.

3 Part 3: CDA vaccination requirements

Only for a foreign-born child aged 12 and below applying for a new pass. Say whether the child holds a Student's Pass from ICA (if so, include a copy of both sides), and enter the CDA reference number from the child's Verification of Vaccination Requirements document.

4 Part 4: Other information

Answer ten questions about the foreigner's history, such as refused entry or deportation, convictions, entering Singapore under another passport or name, previous citizenship or PR, work visas elsewhere (with the country and length), and past study, work or long-term stays in Singapore. Give details for every Yes.

5 Parts 5 and 6: Declarations

The foreigner signs a declaration about the information, health (AIDS, HIV and TB), drugs and consent to verification. A parent signs for a child under 16. The work pass candidate/holder then undertakes responsibility for the foreigner's upkeep and maintenance, and for their compliance with any quarantine and medical surveillance, and signs.

6 Parts 7 and 8: Sponsor and agency

The local sponsor company, which must be the same company that sponsored the work pass, checks Parts 1-6, fills in its details and its representative signs. The representative must be authorised HR personnel or hold at least a managerial position. If an employment agency was used, it completes Part 8.

Common Mistakes to Avoid

  • Using this form when the work pass is not under the EP Sponsorship scheme. Those dependants apply online.
  • Leaving fields blank instead of entering 'N.A'.
  • Writing the name with a salutation (Mr, Miss, Doctor) or differently from the travel document.
  • Choosing a relationship that doesn't fit the child's age, for example 'under 21' for a child who has turned 21.
  • Forgetting the CDA vaccination document for a foreign-born child aged 12 and below.
  • Answering Yes in Part 4 without giving details.
  • A sponsor company different from the one that sponsored the work pass.
  • Missing an English translation for non-English documents.

After You Fill It Out

The foreigner and the work pass candidate/holder sign Parts 5 and 6 and send the form to the local sponsor. The sponsor completes Parts 7 and 8, then uploads the signed form (1 PDF file), the travel document page and the supporting documents (1 combined PDF), and pays the fee at go.gov.sg/submit-dependants-of-ep-sponsorship.

The form says processing takes within 6 weeks, and some cases take longer. Check the status at go.gov.sg/check-work-pass-application after 1 week. MOM may ask for more documents.

VergeHR keeps the completed form in the family member's folder in Files, next to their passport, so the pass and its expiry can be tracked.
