What is Form 1040-V?
Form 1040-V, Payment Voucher for Individuals, is a short statement you send with a check or money order for any balance due on the "Amount you owe" line of your 2025 Form 1040, 1040-SR or 1040-NR. It tells the IRS whose payment it is, which Social Security numbers it belongs to and how much you're paying, so the payment is credited to the right account.
You only need it if you pay by check or money order. The form itself points out that if you pay online at www.irs.gov/Payments, you don't complete it. Electronic options listed on the form include your IRS Online Account, Direct Pay, EFTPS, an electronic funds withdrawal when e-filing, and card payments through a processor (processing fees apply).
VergeHR fills in the voucher from your answers, with your names printed exactly as you enter them, and saves the official IRS PDF in Files. You print it, cut off the voucher, and mail it yourself with your payment; VergeHR never sends payments or forms to the IRS.

When to Use Form 1040-V
- When you mail your 2025 return with a payment, or mail a payment for a return you've filed.
- Not when you pay online, by phone, by card or by electronic funds withdrawal: the form says not to complete it if you pay at www.irs.gov/Payments.
- Not for estimated tax payments for the next year; those use the vouchers in Form 1040-ES.
What You Need Before You Start
- Your Social Security number (or ITIN), and your spouse's if it's a joint return.
- The amount you're paying.
- Your name(s) and address exactly as shown on your return.
- A check or money order payable to "United States Treasury".
What’s On Form 1040-V
The 2025 edition has 2 pages. VergeHR asks for it in 2 parts:
- Lines 1 to 3: Your paymentTaxpayer
- Line 4: Name(s) and address, exactly as shown on your returnTaxpayer
How to fill out Form 1040-V, step by step
1 Line 1: Your SSN
Enter your Social Security number. For a joint return, enter the SSN shown first on the return.
2 Line 2: Second SSN
For a joint return only, enter the SSN shown second on the return.
3 Line 3: Amount
Enter the amount you're paying by check or money order.
4 Line 4: Names and address
Enter your name(s) and address exactly as shown on your return, including the apartment number. For a foreign address, fill in the country, province or county, and postal code as well.
5 Prepare the check
Make the check or money order payable to "United States Treasury". Put your name and address on it, and your daytime phone number, SSN (the one shown first on a joint return) and "2025 Form 1040", "2025 Form 1040-SR" or "2025 Form 1040-NR" as appropriate. Write the amount on the right side like $ XXX.XX, without dashes or lines.
Common Mistakes to Avoid
- Stapling or clipping the voucher or payment to the return. The form says to put them loose in the envelope.
- Sending cash. Cash can only be paid in person through the IRS's retail payment option described on the form.
- Writing the amount on the check as "$ XXX—" or "$ XXX xx/100". Use $ XXX.XX.
- Putting the SSNs in a different order from the joint return.
- Filling in the voucher when you've already paid online. The form says not to complete it in that case.
- Sending a single check for $100 million or more. The IRS can't accept one; split it across several checks.
After You Fill It Out
Print the PDF, cut the voucher off along the dashed line, and mail it with your check or money order (and your 2025 return, if you're mailing that too) to the address on page 2 that applies to where you live: page 2 lists separate addresses by state, and one for foreign addresses, American Samoa, Puerto Rico, APO/FPO addresses and certain other filers.
Keep a copy of the voucher in VergeHR Files and a record of the check. When you pay by check, the IRS may convert it into a one-time electronic funds transfer, so the money can leave your account as soon as the day the payment arrives and the check won't come back from your bank.
