What is a product testing consent?

Home-use tests are how brands learn whether a lotion irritates, a gadget confuses or a snack actually tastes good. Before a prototype leaves the building, testers need to know what is in it and what the risks are, and the company needs to know the product will not appear on social media before launch. A product testing consent covers both.

This template sets out the product, the test dates and how to use it, then the safety information — ingredients or materials, known risks and who should not take part — with extra wording for unreleased prototypes. The tester confirms they are an adult, agrees to stop and report any reaction, and gives honest feedback by questionnaire, diary or interview. You choose whether comments stay internal or may be quoted anonymously or with a first name and initial. Confidentiality is strict for prototypes and lighter for products already on sale. It also covers any reward, whether the tester keeps, returns or disposes of the product, personal information and reaction reports, and it does not exclude the company's responsibility for defects or consumers' legal rights.

  7. Withdrawing and general terms

Using the product testing consent, step by step

1 Describe the product and how to use it

Give the product or project name, what it is, the test dates and exact instructions — including any patch test or dosage. The tester agrees to use it only personally and as instructed.

2 Disclose the safety information

List the ingredients or materials, known risks and allergens, and who should not take part. This is the most important part of the form: a tester can only give real consent if they know what they are trying.

3 Say whether it is a prototype

For an unreleased prototype, the form explains that it may not have had every test a finished product receives, and applies full confidentiality. For a product already on sale, only unreleased information stays private, and reviews must say the product was received free.

4 Set feedback, reward and return

Choose how feedback is given and whether comments can be quoted publicly. Add any reward, and whether the product is kept, returned in prepaid packaging, or disposed of as instructed.

5 Send to each tester

Each tester initials each page and signs before their sample ships. Keep the signed forms with your test records.

Tips Before You Send It

  • Screen out people in the "do not take part" group before shipping, not after — the form confirms it, but screening prevents problems.
  • Give testers one clear contact email for reactions, and actually monitor it during the test.
  • Regulated products such as medicines, medical devices or anything ingested with health claims may need formal study approvals — this form is for ordinary consumer testing.
  • If testers will post reviews, many advertising rules require them to disclose free products. The form says so for products already on sale.

Product testing consent: frequently asked questions

Can I use this for a clinical trial?

No. Clinical trials and studies of medicines or medical devices are governed by specific laws and ethics approvals. This template is for ordinary consumer product testing.

Who owns the feedback?

The company may use the feedback, and any ideas in it, freely to develop and market its products, without payment beyond any reward in the form. Whether comments can be quoted publicly is a separate choice.

What if a tester has a reaction?

They must stop using the product, seek medical advice where appropriate and tell the company as soon as possible. The company records every report and may share reaction reports with regulators where the law requires.

Is an electronic signature valid for a consent like this?

Generally yes. Electronic signatures are valid for ordinary agreements in the US (ESIGN and UETA), the UK, the EU (eIDAS) and Canada. VergeHR keeps an audit trail and Certificate of Completion with the signed PDF.
