What is a salon service consent?

Before a colour, lash, brow, waxing or nail treatment, a good salon asks about allergies and health, carries out a patch or sensitivity test where the products call for one, and explains the aftercare. A salon consent form (sometimes called a client consultation form or patch test record) puts all of that in writing: what the client told you, what the test showed, and that they understood the risks and agreed to go ahead.

This template records the treatment, products and technician, the client's allergies and relevant health points (such as pregnancy, skin conditions, retinoid medication or recent peels), and the patch test result — clear, reaction noted, declined by the client, or not needed. Each result prints its own clause; if the client declines a recommended test, they acknowledge the increased risk and the salon may still refuse. It explains realistic results, what's normal during a treatment, and what to do about a reaction — including getting emergency help for severe symptoms. Aftercare switches to match the treatment you choose, and there's a before-and-after photo choice. For clients under 18, a parent or guardian answers and signs.

  5. Before and after photos

Using the salon service consent, step by step

1 Choose the treatment

Pick the treatment and add details and the main products — for example "full head balayage with toner". The aftercare section fills in automatically with advice for that treatment, and you can add your own notes.

2 Record health and allergies

Enter any allergies and anything that could affect the treatment. Leave them empty and the form records "none known". The client confirms the information is accurate and agrees to tell you if it changes.

3 Record the patch test

Choose the result and add the date and any notes. If the client declines a recommended test, the form records that they understand the increased risk and that the salon may still refuse the treatment.

4 Sign at the consultation

Send the signing link to the client's email so they can open it on their phone at the consultation. They sign, and you sign as the technician. The form covers repeat treatments of the same kind until their information changes.

Tips Before You Send It

  • Follow the product manufacturer's instructions on patch testing — many hair colour brands call for a test before every colour service.
  • Update the form whenever the client starts new medication, becomes pregnant or has a reaction.
  • Explain aftercare out loud as well; the form summarises it, but clients remember what they're told.
  • Only post before-and-after photos when the client has chosen the portfolio option.

Salon service consent: frequently asked questions

Does a signed consent form protect the salon from all claims?

No form can do that. The template says nothing limits the salon's responsibility for its own negligence or liability that can't be limited by law. What it does is record the information you relied on, the test result and the advice you gave.

What if a client refuses a patch test?

Choose "Not done — the client declined". The form records that the test was recommended, that the client understands the risk of a reaction without it, and that the salon may still decline the treatment.

Can I use it for clients under 18?

Yes. Choose "Under 18" and enter the parent or guardian's name; they answer for the client and sign. Check your salon's policy and any local rules on treating minors, especially for waxing and chemical services.

Is an electronic signature valid?

Electronic signatures are generally valid for consent forms like this in the US (ESIGN Act and UETA), the UK, the EU (eIDAS) and Canada. VergeHR keeps an audit trail and a Certificate of Completion.
